My Life, My Health: The Chronic Disease Self-Management Program

Mount Auburn Hospital presents a free six week workshop called “My Life, My Health.” This program is offered to adults living with, or caring for, someone with a chronic health condition. Participants can learn how to set and meet personal goals, identify and try out simple techniques to improve quality of life, gain skills to improve patient-to-doctor communication, learn to manage stress, raise energy levels, make educated choices about treatment, and have fun!


The Chronic Disease Self-Management Program was developed by researchers at Stanford University Medical Center and has been proven to help people just like you! Participants will receive a copy of the book, “Living a Healthy Life with Chronic Conditions,” as well as a relaxation CD.


The program is available on November 5th, 19th, and 26th. As well as December 3rd and 10th. Sessions will run from 9:30am-12:00pm and refreshments will be provided.
